As a requirement for entry onto the course, you will be required to complete a suitability declaration form. We expect you to be open and honest when coming onto a health and social care course, as per your professional body expectation. Failure to disclose and the information becoming available at a later opportunity will see you referred through Fitness to Practise proceedings which may lead to removal from the course.
Please provide as much information as possible and this will help any consideration of disclosures. If you are unsure what you are required to disclose you can find further guidance from Unlock. All disclosures are considered on a case-by-case basis and disclosure does not automatically mean that you cannot enter the course.
