You are required to complete an online Health Questionnaire to determine your suitability to train to teach. This confidential process is in collaboration with our NHS Occupational Health partner, Sheffield Teaching Hospitals.

You will receive two emails from notifications@cority.com. These emails will be sent to you shortly, the first email will contain your account information and the second email will contain your one-time password, both emails will include a link to the questionnaire you need to complete. In our experience, it’s likely the email will be delivered to your Junk/Spam folder, so please check.

For Overseas applicants only: If our healthcare provider requires additional information, they will request this by email. If the nature of your declaration needed an actual conversation, our healthcare provider would either do this by telephone or by their clinic video consultation system “Attend Anywhere” which is like Zoom.
